To get started, please complete the following steps for one of the options
Option 1: Illuminate Portal
1) Log into your Illuminate Parent Portal (Your form will be waiting for you when you log in).

2) Locate the "Forms" tab on the left
3) Click on Home Income Form and complete
Option 2: Downloadable Adobe Document
You can follow the link provided to download an electronic copy to fill out, save, and send in to [email protected]
Option 3: Paper copy filled out at NPHS
YOU HAVE THE OPTION TO FILL OUT THE PAPER FORM AT NPHS BETWEEN THE HOURS OF 8am-4pm Monday through Friday.
